Physical Therapy Board of California
KEMG/ENMG Certification Applicant
Fingerprinting
You must fingerprint for the PTBC even if you have already fingerprinted for another agency.
Live Scan – Required if in California
If you will be in California at any time during the application process, or if you are a California resident, submit your fingerprints via a California Live Scan location. Answer ‘No” to the fingerprinting hard card question when completing your online Breeze application. A Live Scan form preprinted with the Board’s DOJ/FBI routing information will be emailed to you at the email address you provide with your application’s Address of Record.
Take the Request for Live Scan Services form to an authorized California Live Scan operator. Make sure the Live Scan operator requests fingerprint processing for both DOJ and FBI. The Live Scan operator will keep the original Live Scan form and give you a copy. Live Scan fingerprinting fees are due directly to the Live Scan Operator at the time of printing. Live Scan fingerprint processing fees are $32 for DOJ and $17 for FBI plus a $25 (on average) rolling fee. Contact the Live Scan location to determine the rolling fee imposed at that location.
You must submit your copy of the Live Scan form to the PTBC upon request.
Hard Card
If you are an out-of-state applicant and will be in California at any point during your application process, the PTBC strongly recommends waiting until you are in California to fingerprint using Live Scan. Live Scan is only available in California, but you do not need to be a California resident. If you will not be in California at any point during your application process, you must use a hard card.
To obtain a fingerprint card packet from PTBC, answer “Yes” to the fingerprinting hard card question when completing your online Breeze application. A fingerprint hard card packet will be sent to you via USPS to your application’s Address of Record within two weeks of payment of application fees; the application fees will include the hard card processing fee of $49 ($32 for DOJ and $17 for FBI).
The packet contains two hard cards, a sample sheet for filling out the cards, a Live Scan form with Privacy Notice, and an Instruction sheet. Take both cards to an authorized fingerprint roller. Do not roll your fingerprints yourself; DOJ will reject them. Return both the completed fingerprint cards and signed Live Scan form (certifying you received fingerprint privacy notices) to the PTBC; the PTBC will forward to the DOJ/FBI for processing.
The completed Live Scan form is required for your application.
