License Renewal

For all license renewals with expiration date of January 31, 2006 and forward, the biennial (two year) license renewal fee is now $150.00. For all license renewal with expiration dates of December 31, 2005 or prior, the biennial (two year) license renewal fee is $120.00. Along with renewal payment, criminal conviction disclosure. It is illegal to practice with an expired license. Licensees found to be practicing with an expired license are subject to disciplinary action. Additionally, a delinquent fee of $60.00 will be added to your renewal fee if not paid within 30 days of the expiration date (Business & Professions Code 2688; California Code of Regulations, Title 16, Section 1399.50 and 1395.52). California does not offer "inactive status" for licensees.

PLEASE NOTE: If a renewal payment is submitted without the required criminal conviction disclosure information, the PTBC, by law, is unable to complete a renewal application. This requirement is located in Section 2684 Business and Professions Code. Please be advised all license renewals are not completed until the criminal conviction disclosure information is submitted. Submitting a renewal payment without the required disclosure constitutes an incomplete license renewal application and will delay the processing of your license renewal.

Continuing Education is not required to renew physical therapist or physical therapist assistant licenses.

Application for Examination and/or Licensure

This link is to request a fingerprint card ONLY if you reside outside of California and are unable to have your fingerprints processed via LiveScan. Please be advised that fingerprint cards can take 3-6 weeks for processing. The Board strongly encourages waiting until you are in California to process your fingerprints via LiveScan as this can minimize the processing time of your application. You may mail your application prior to processing your fingerprints via LiveScan if you are soon to make the move to California.

The application is not available by mail. To receive the application, please follow the instructions below on how to apply for a license and download the application.

The Board will not mail an application packet to you as the current application is available on the website. Please indicate your name, mailing address, and phone number to receive the fingerprint card. If you do not provide the required information, your request will not be processed. Please allow up to two weeks to receive the fingerprint card. The Reference Guide to the Laws and Regulations Governing the Practice of Physical Therapy is not available by mail. You will need to download the Reference Guide to the Laws & Regulations Governing the Practice of Physical Therapy.

Accredited Physical Therapist Applicants

This link is for applicants that have graduated from an accredited physical therapy program and currently have an application on file. If you graduated from a physical therapy program in a foreign country, please verify that your program is considered an accredited program through the American Physical Therapy Association's (APTA) web site at www.apta.org/AM/. Currently the only countries that have accredited programs are Canada, France, Northern Ireland, Scotland, and the Netherlands.

Foreign Non-accredited Physical Therapist Applicants

This link is for applicants that have graduated from a physical therapy program in another country that is not accredited by the APTA and currently has an application on file. All inquiries regarding foreign educated physical therapist that graduated from a non-accredited physical therapy program should be directed to this link. In your e-mail, please include the applicants name if you are writing on behalf of an applicant.

Physical Therapist Assistant Applicants

This link is for all applicants that have a physical therapist assistant application on file. This includes equivalency applicants, foreign educated applicants that are applying for a physical therapist assistant license, and graduated of an accredited physical therapist assistant program.

All name changes must be completed on the required Name/Address Change and/or Duplicate Wall/Wallet request form.

You can now change your address online by clicking here. If you are unable to change your address online then you must complete the required Name/Address Change and/or Duplicate Wall/Wallet Request form above. This form can be mailed or faxed to the PTBC.
