What Makes an Application Complete
A Kinesiological Electromyography (KEMG) Application must contain the Following Documents
- The Fee Schedule form with the appropriate application and examination fees.
- Application with all questions answered, picture affixed, signed and dated.
- Completion of the Affidavit of Self-Study for KEMG form or official transcripts documenting completion of the required course work pursuant to section 1399.63 of Title 16 of the California Code of Regulations.
- Affidavit of Training and Supervision for KEMG form - signed by the supervisor.
- Agency Copy of Live Scan form for the Federal Bureau of Investigation (FBI) only.
- If you require reasonable accommodations for the examination, a completed Disability Accommodations Request Form (D1) must be completed and submitted with the application.
A Electroneuromyography (ENMG) Application must contain of the Following Documents
- The Fee Schedule form with the appropriate application and examination fees.
- Application with all questions answered, picture affixed, signed and dated.
- Completion of the Affidavit of Self-Study for ENMG form or official transcripts documenting completion of the required course work pursuant to section 1399.64 of the California Code of Regulations.
- Affidavit of Training and Supervision for ENMG form – signed by the supervisor.
- Agency Copy of Live Scan form for the Federal Bureau of Investigation (FBI) only.
- If you require reasonable accommodations for the examination, a completed Disability Accommodations Request Form (D1) must be completed and submitted with the application.
Failure to submit a complete application may result in your application being closed.
