Prior to submitting an e-mail inquiry to the PTBC, please be thorough in researching the information on the PTBC’s web site. The PTBC has made every effort to ensure that the information you are seeking is included in this web site. Due to the large volume of inquires and staff workload the PTBC appreciates your efforts in researching the information prior to contacting board staff.
CONSUMER PROTECTION SERVICES
Consumer Protection Services representatives can assist in the filing of a complaint and answer questions pertaining to discipline or a citation.
If you are interested in applying to serve as an Expert Consultant for the PTBC, contact the PTBC's office to speak to a Consumer Protection Services Representative.
APPLICATION & LICENSING SERVICES
The Application & Licensing Services unit responds to e-mails relating to verification of a license, license renewal, and application questions. If you are an applicant or licensee and need to notify the PTBC of your address or name change, please click here.
License Renewal Information – License renewal information is located under Licensees on the top navigation bar on the PTBC’s web site.
Applicant & License Inquiries – If you have a question relating to your application on file or your license, please include in the e-mail your name, address, telephone number, and type of application or licensee number.
PTA@dca.ca.gov - Submit your inquiry to this e-mail address if you are a licensee or a graduate of an accredited PT, PTA Program or PTA Equivalency applicant.
FPT@dca.ca.gov– Submit your inquiry to this e-mail address if you are a foreign educated PT or PTA applicant.
Application & Fingerprint Card Request – The PTBC will mail the fingerprint card ONLY if you reside outside of California and are unable to have your fingerprints processed via Live Scan. Please be advised that fingerprint cards can take 3-6 weeks for processing. The PTBC strongly encourages waiting until you are in California to process your fingerprints via Live Scan as this can minimize the processing time of your application. You may mail your application prior to processing your fingerprints via Live Scan if you are soon to make the move to California.
The application is not available by mail. To receive the application, please follow the instructions below on how to apply for a license and download the application.

The PTBC will not mail an application packet to you as the current application is available on the web site. Please indicate your name, mailing address, and phone number to receive the fingerprint card. If you do not provide the required information, your request will not be processed. Please allow up to two weeks to receive the fingerprint card. The Reference Guide to the Laws and Regulations Governing the Practice of Physical Therapy is not available by mail. You will need to download the Reference Guide to the Laws & Regulations Governing the Practice of Physical Therapy.

ADMINISTRATIVE SERVICES
Practice Issues – If you have a scope of practice inquiry that is undefined in the Physical Therapy Practice Act, you must submit a written inquiry to the Board. The inquiry must be requested in the format as instructed on the Practice Issues section located under Laws/Regulations in the top navigation bar on the PTBC’s web site.
Outreach, Diversion, Board Meetings, etc. – Please contact the PTBC to speak to a representative in the Administrative Services Program.
